Why this review matters
Most maternity services have numerous listening routes: surveys, FFT, complaints, PALS, MNVP engagement, birth reflection, family liaison, incidents, claims, staff conversations and informal feedback. Yet these sources often sit in different teams, use different categories and reach decision-makers through separate governance routes.
This can create the appearance of extensive listening without a coherent view of what women and families are saying, what it means for safety, or whether anything changes as a result.
The Maternity Listening, Learning & Safety Review examines the whole pathway from voice to action. It is not a satisfaction survey and not a generic culture audit. It asks whether the organisation can recognise experience-related risk, respond compassionately, learn across sources and demonstrate improvement.

Whether women and families experience curiosity, continuity, explanation, transparency and appropriate resolution — or repetition, defensiveness and hand-offs.
Intelligence and triangulation
How surveys, narrative feedback, complaints, PALS, incidents, claims, birth reflection and community insight are coded, connected and interpreted.
Governance and accountability
Where themes are discussed, who owns action, how family perspectives enter safety responses, and whether leaders receive a meaningful account rather than disconnected metrics.
Learning and improvement
How insights become priorities, interventions and measurable change — and whether the organisation closes the loop with families, staff and the Board.
Workforce capability and culture
Whether staff have the time, confidence, skills, psychological safety and support to listen and respond well, particularly following harm or conflict.
